Name change

To update your personal details, you will need to complete a Change of Name Form and send it to us via one of the following methods.

Please note that if you have changed your name, we will need to see a copy of your marriage certificate or the deed poll.

*Please note that we are only able to discuss details of an agreement with the agreement holder. If you wish to discuss a partner/family member’s agreement with us, we will need the customer’s expressed authority to do this. We will require the customer to contact us on 0370 5050 125 and provide details of the third party’s name, date of birth and will ask for a password to be provided, which will be used to identify them. **Lines are open 8.00am to 7.00pm Monday to Friday and 9.00am to 5.00pm Saturday and 10.00am to 4:00pm Sunday. Calls may be recorded for training purposes.
